Correcting for Pelvic Posture Influence on Cup Anteversion Measurements

چکیده
INTRODUCTION Malposition of the acetabular component is a risk factor for postoperative dislocation after total hip arthroplasty (THA)[1]. Current radiographic techniques for assessing cup anteversion have been developed, but they have limitations[2]. The purpose of this study is to investigate the effect of and reduce the error caused by various pelvis postures on cup anteversion measurement. We hypothesize measurement of cup anteversion has a strong linear relationship with pelvis tilt or rotation. Applying techniques that measure pelvis posture (tilt or rotation) correction parameters (CP) for cup anteversion can be calculated and the accuracy of cup anteversion assessment can be improved[3, 4]. METHODS Six subjects were recruited to receive pelvis CT scans with IRB approval. With CT data and cup CAD models as input, Analyze, a medical imaging software, was applied to create a 3D pelvis model. The pelvis model was then realigned according to Lewinnek’s study, which suggested orienting the anterior pelvic plane (APP) parallel to coronal plane. Cup placements were simulated in 15 positions relative to APP in every subject, including cup inclination of 30°, 40° and 50° coupled with anteversion of -5,° 5°, 15°, 25° and 35°, respectively (Figure 1).

Measuring Acetabular Cup Orientation on Antero-Posterior Radiographs of the Hip after Total Hip Arthroplasty with a Vector Arithmetic Radiological Method. Is It Valid and Verified for Daily Clinical Practice?
PURPOSE The aim of this prospective study is to validate a vector arithmetic method for measuring acetabular cup orientation after total hip arthroplasty (THA) and to verify the clinical practice. MATERIALS AND METHODS We measured cup anteversion and inclination of 123 patients after cementless primary THA twice by two examiners on AP pelvic radiographs with a vector arithmetic method and com...
متن کاملInfluence of Bilateral and Unilateral Flatfoot on Pelvic Alignment
Background: The change in foot posture can possibly generate changes in the pelvic alignment. There is still a lack of evidence about the effects of bilateral and unilateral flatfoot on possible changes in pelvic alignment. The purpose of this study was to investigate the effect of flatfoot on the sagittal and frontal planes of pelvic postures. Materials and Methods: 56 subjects, aged 18–40 yea...
متن کاملInfluence of Surgical Approach on Pelvic Lift in Hip Arthroplasty During Cup Insertion.
The position of the acetabular cup is a major factor in the long-term outcome of total hip arthroplasty (THA). Malpositioning of the acetabular cup frequently has been reported with the use of a minimally invasive implantation technique. It remains unclear whether the limited visibility or the increased retractor traction and thus tilting of the pelvis during cup implantation is the cause. This...
متن کاملValidation of a new method for determination of cup orientation in THA.
Our goal was to validate accuracy, consistency, and reproducibility/reliability of a new method for determining cup orientation in total hip arthroplasty (THA). This method allows matching the 3D-model from CT images or slices with the projected pelvis on an anteroposterior pelvic radiograph using a fully automated registration procedure. Cup orientation (inclination and anteversion) is calcula...
متن کاملMorbidly Obese Patients Undergoing Reduced Cup Anteversion Through a Direct Lateral Approach.
BACKGROUND The presence of obesity negatively affects the results after total hip arthroplasty. The influence of morbid obesity on cup positioning is investigated. METHODS A retrospective analysis of radiographs from 790 patients in 2013 and 2014 was performed. The correlation of cup inclination and anteversion with body mass index (BMI) was analyzed. Three groups were formed: 139 patients wi...
